Welcome to the Childrens Advancement Fund

Clients

  1. Clients must have a need based on standardized testing. Scores should demonstrate a total test score one standard deviation below the mean or a percentile rank of 16 or less.
  2. Clients must qualify for funding by following under income guidelines. Families will be asked to submit financial documentation (personal and business tax return and investment income).
  3. Clients are able to request that their provider participate in Children's Advancement Fund (CAF).
  4. All children will be placed on one waiting list and families will be contacted as space is available.
  5. Children will be discharged when progress plateaus or gains are not noted across a 6 month period. Progress notes that do not show progress towards goals at the first 3 month review will be discussed with parents and provider prior to discharge from program.
  6. Clients agree to keep all scheduled appointments. If more than a total of 15 % of appointments are missed per quarter to include scheduled cancellations the client will be discharged.
  7. If funds are not available to continue therapy services the client will be notified within 30 days of termination of services.
  8. The clients agree to one visit a week in each discipline that the child qualifies for.
  9. Clients agree to follow providers cancellation and no show policies. Cancellation and no show fees cannot be submitted to CAF.
